How to describe a printable form?

If a unique form ID is used for the printable form, enter it here. The maximum length for a form ID is 100 characters. For example, the form ID that identifies Kela’s child benefit application is LL 1.

Click the Add new form file button.

First, use the drop-down menu to select the file format of the form. If a form is available in several different file formats, enter the information for each file format separately by clicking the Add a new form file button. Select the file format for each form and enter a unique web address. Accepted file formats include PDF, DOC/DOCX, XLS/XLSX, RTF and ODT.

Next, type in the web address of the form. The address has to start with http:// or https://. The system will return an error if you enter an address without this initial part. 

The printable form has to be located at an external network address, for example a server owned by the organisation. You cannot save or upload form files to the Suomi.fi Finnish Service Catalogue.

Click "Accept".

If the address or other information concerning where to deliver the form is clearly visible on the form itself, you do not need to fill in this section.

If you want to enter information about where to deliver the form, click the Add new delivery option button.

Form recipient

The form recipient is optional. You can enter the name of the receiving organisation and any details about the recipient of the form, for example the name of an individual or unit.

Type of postal address  

You can enter the delivery information with the Type of postal address field by specifying either the street address or P.O. box where the customer should send the form, or you can choose to enter the delivery information in your own words. You could for example write “Fill in the form and bring it, signed by guardian, to the instructor at Naakkakerho when you go for the first time”. 

Note! Describing the delivery address or other address information in just the user instructions of the service is not enough because the customer might not see the user instructions when going to the service channel information. A customer may also arrive on the printable form page without visiting the service page at all. 

If the printable form includes attachments or additional information pages that are on a website outside the Suomi.fi Finnish Service Catalogue, you can enter their details under “Appendix and additional information links", which is the lowest item under Service channel details. You can enter multiple web addresses.

The attachment file or additional information page should be located at an external web address because no attachment files can be uploaded or saved in the Finnish Service Catalogue.

Start by clicking the "Add new web address" button to open the fields to fill in.

Name of the website

In this field, enter a customer-oriented name that describes the website.

Web address

In this field, enter the exact web address where the attachment or additional information page opens up. The address has to start with http:// or https://. The system will return an error if you enter an address without this initial part.

Description (optional)

If necessary, enter a description of the purpose of the attachment file or link to additional information.

If you want to enter more than one website with attachments or additional information, click the Add new web address button below the section.

If you want to delete a website you have entered in the Attachment and additional information links section, click the Delete button in the bottom right corner.

Note! If a form can be used in different languages and you want to describe the printable form in the Suomi.fi Finnish Service Catalogue in these other languages as well, remember to enter the exact address of each language version.

When you have filled in the information, click the "Save as draft" button at the bottom of the page.

Finally, if you are ready to publish the content, click the "Publish" button at the top right of the page and follow the steps for publishing.
